Student’s Official State Birth Certificate
Student’s Immunization Record
Student’s Social Security Number
2 Proofs of Residency (rental/lease agreement, rent receipts, closing statement, utility bill)

Attendance Policy

In grades K-8, a student must be present 90% of the quarter. Except for suspensions and school activity, religious holy days and confirmed illness, all absences will be counted. If a student is absent more than 10% of the quarter, the following actions will be taken:

Students will receive no grades for the quarter. Report cards will reflect N.G. (No Grade). Students will be expected to attend classes and complete course requirements for the remainder of the quarter.
A referral will be made to the appropriate authorities.
Retention will be considered by the Site Attendance Committee.

A letter will be sent from the site attendance officer to the parent upon the fifth unexcused absence in a four-week period. The letter will state the number of absences and will inform the parent that he/she must contact the school concerning these absences or return the signed Affidavit of Understanding. A copy of the letter will also be sent to the appropriate authorities.

Students who are absent from school 15 or more days or "parts of days" (tardies/earlyouts) within a semester or 4 or more days or "parts of days" within a 4 week period without valid excuse shall receive a letter from the school. Failure to contact the principal or attendance officer and/or failure to correct the attendance problem shall result in a referral to the appropriate authorities.

Absences & Tardies

ALL GRADES: Telephone the school office at 366-5932 any time your student will be absent or more than 15 minutes late. The school attendance officer will try to phone you at home if your student is absent. If absence or tardy is due to illness or a visit to the doctor’s office, please provide medical verification to excuse the absence/tardy.

PRE-K & HALF DAY KINDERGARTEN: Hours are 7:55-10:25 a.m. Arrival after 7:55 and before 8:25 is a tardy. Check out after 9:55 is an early out. Arrival after 8:25 and departure before 9:55 is an all day absence.

FULL DAY KINDERGARTEN & GRADES 1-5: Hours are 7:55-2:40. Arrival after 7:55 and departure before 2:40 is considered a tardy/early out. Arrival after 8:55 and departure before 1:40 is a ½ day absence.

Students in grades K-5 may be signed out for lunch/recess by a parent only during the following times:

K & 1^st 11:00-11:45

2^nd & 3^rd 11:30-12:15

4^th & 5^th 12:00-12:45

Checking out earlier or arrival back at school later will be a tardy. Please sign student back in when returning to campus.

Attendance Awards

All absences (excused & unexcused) count towards attendance awards.

Class attendance parties will be given for 25, 50, 75 & 100 days perfect attendance.
Students will be awarded a ribbon for perfect attendance each 9 weeks.
Students with 98% attendance for the year will be awarded a certificate on the last day of school.
Students with perfect attendance for the year will be awarded a trophy on the last day of school.

Schedule

7:25 Teachers arrive & doors are unlocked.

7:25-7:45 Breakfast served in the gym.

7:25 Students report to the gym for breakfast or line up in grade level lines.

7:25-7:40 Teachers planning time.

7:40 Students are dismissed from gym to report to classrooms.

7:55 Class begins.

11:00-11:45 Lunch/Recess for K & 1^st.

11:30-12:15 Lunch/Recess for 2^nd & 3^rd.

12:00-12:45 Lunch/Recess for 4^th & 5^th.

7:55-10:25 School hours for morning Pre-k & Half Day Kindergarten.

12:10-2:40 School hours for afternoon Pre-K (if applicable).

2:40 School is dismissed.
